The Truth About Dermal Fillers and Under-Eye Puffiness

Dermal fillers are a quick and minimally invasive way to reverse the signs of aging around the eyes, nose, and mouth. One of the most popular filler procedures is the eye lift to reduce the appearance of bags and dark circles under the eyes. “When performed properly, these fillers start to eliminate fine lines, creases, and bags under the eyes almost instantly,” explains Dr. Miriam Hanson, dermatologist and cosmetic expert in Austin, Texas. While temporary puffiness after an under-eye filler injection is normal, puffiness that does not resolve may require further treatment.


Causes of Under-Eye Puffiness After Filler Treatments

The skin under the eyes is especially thin, so the injection must be handled with care and precision to achieve the best possible results. Swelling is common in this area, and can sometimes be prolonged especially if the patient has allergies or other contributing factors. The following are a few of the reasons why under-eye fillers can cause unintended puffiness.


  • If the filler is placed too superficially, the skin at the injection site will have a blue tint. This is known as the Tyndall effect. Placing the filler deeper into the skin will keep it from being visible.
  • It is possible to have too much of a good thing. “If too much filler is used, you will lose the normal eye contour, and there will be noticeable pouches under the eyes,” Hanson shares.
  • Different fillers are designed for different applications. Even though many fillers are made from hyaluronic acid, some have a greater caliber and are more robust than others. Fillers like Juvederm Voluma are better suited for cheeks and mid facial rejuvenation compared to Vobella and Restylane Silk which perform better under the eyes.
  • Incorrect filler choice or placement can exacerbate the appearance of the cheek bags known as festoons, which can contribute to under-eye puffiness.
  • As a general rule, permanent fillers, including Sculptra, should not be used under the eyes. Some patients experience reactions to these fillers and can develop lumps or granulomas at the injection site.
Treating Under-Eye Puffiness Following a Filler Injection

Following filler injection, you can keep swelling to a minimum with warm compresses and light massage and by sleeping with your head elevated. Puffiness that is not the result of normal swelling can typically be reversed. “An injection of an enzyme will help dissolve the filler and eliminate the unwanted puffiness,” says Dr. Hanson. Permanent fillers are more challenging to correct and may require a surgical procedure.
